Tell us you’re interested in having an Illinois 4-H license plate.
- We need contact information from a minimum of 1,500 individuals to qualify for having the 4-H license plate produced. Sign up below.
- After we reach the 1,500 goal, a 4-H license plate order form will be sent to you, along with further instructions on ordering.
- Your order form and deposit¹must be submitted to the Illinois Secretary of State’s office – this serves as a commitment for purchase of the 4-H license plate.
- Upon the Secretary of State’s office receiving 1,500 orders, the license plate will be manufactured and ready for purchase².
